Our mission is to try and reduce the health disparities that cause chronic conditions, but we acknowledge that there is still a huge amount of work to do in supporting people who are suffering from existing chronic diseases.

There are many voluntary organisations dedicated to supporting specific chronic conditions, who we would welcome hearing from.

We are interested in supporting voluntary organisations that offer support services to individuals suffering from chronic disease and their families. This is the type of work will consider supporting:

Organisations that offer emotional support to individuals and families dealing with chronic illness.
Projects that provide financial and practical advice around a specific chronic disease.
Organisations that offer care services to people suffering.
Organisations researching new ways to deal with chronic illness
Hospices.

We would love to hear from projects that focus on individuals dealing with chronic illness and their families.
